The Grey’s Anatomy franchise is officially heading south. ABC has greenlit a brand-new, currently untitled medical drama set in the rugged landscape of West Texas, as recently confirmed by Deadline.
The show is billed as an “edgy drama about a team at a West Texas rural medical center — the last chance for care before miles of nowhere.”
Here is what you need to know about the highly anticipated spin-off:

Original Grey’s Anatomy creator Shonda Rhimes is teaming up with current showrunner Meg Marinis to co-create, write, and executive produce the project. Franchise star Ellen Pompeo is also on board as an executive producer.
For Marinis, a Houston native and University of Texas at Austin alumna, the setting is deeply personal. “I am incredibly excited to expand the Grey’s Anatomy universe,” she stated. Marinis emphasized that the new show will capture the “heart, emotion and connection” fans have cherished for over twenty years, all against the backdrop of her home state.
Unlike previous franchise extensions (Private Practice, Station 19, and Grey’s Anatomy B-Team), this fourth spin-off will not be anchored by an existing character crossing over from Grey Sloan Memorial. Instead, it will introduce an entirely new ensemble cast. Any potential narrative ties to the original beloved characters remain tightly under wraps.

The series is slated for a midseason premiere in the 2026–2027.
While fans wait for the official release date and title, they can catch up on the flagship series. Grey’s Anatomy, which just wrapped its 22nd season this May, is currently available to stream on Hulu.
